Letter of Recommendation Requests
If
you had courses with Dr. Malott and think he may have good
things to say about you, then you may request a letter of
recommendation for grad school, scholarships, etc.
You
will need this form:
LOR Request Form (Word
document)
Forms are also available on the door to the BATS lab (2536
Wood Hall) and in the course packs for any of Dr. Malott's
classes.
Return the completed form with ALL of the materials listed
to:
Graduate mailbox "L"-in the Psychology Department's
main office (3rd floor of Wood Hall),
Mailbox labeled "Jennifer" in the BATS Lab (2536 Wood
Hall),
Or mail the materials to either of the following addresses:
